Refund Policy

The purchaser of a Fisher Wallace Stimulator® may return the device for a refund within 30 days of receiving it. Shipping charges and add-on accessories such as extra sponges will not be reimbursed, and the customer is responsible for shipping the device to us. Fisher Wallace applies a restocking fee that is 10% of the amount paid for the device. The restocking fee covers the cost of testing and sanitizing the device, disposing of and replacing accessories, and repackaging the unit for use in research and charitable distribution.

You can expect your refund within four to five weeks of Fisher Wallace receiving the returned device. If the returned device is damaged, no refund will be processed. Please make sure to use delivery confirmation when shipping a device back to Fisher Wallace Laboratories. Fisher Wallace is not responsible for refunding the purchase of a device that is not returned or for which no return delivery confirmation is available.

It typically takes four to six weeks for Fisher Wallace to process a refund after receiving the returned device. If you do not receive your refund check within five weeks please contact Fisher Wallace.

Finance your purchase - Select During Checkout

Finance your purchase with for as low as at $27.64 per month (Based on a $599 purchase, at 9.9% APR for 24 months. Rates based on credit). Applying will not affect your credit score. You may return your device for a refund within 30 days of receipt. Shipping and extra accessories are non-refundable and a restocking fee is applied.

Returning Your Device for a Refund

If you are confident that you want to return the Fisher Wallace Stimulator for a refund, please read over and fill out the Return Form.

Please follow the packaging and return instructions on the form. No return authorization number is required for your return.

You may use any carrier to ship your device back to us, but we strongly recommend you use delivery confirmation. We are not responsible for refunding a lost device if there is no proof that we received it.

Your refund will come in form of a check in 4-6 weeks from the date we received your return.

Repairing Your Device

If you are having difficulty using your device, please read the troubleshooting section (12.0) of the instructional manual, where you will find solutions for the most common problems. If you still need help, please call us at 800.692.4380. If we cannot solve your problem over the phone, we will repair or replace your device at no charge, so long as it is still under warranty. You will be responsible for shipping your device to us.

Flexible Spending Account

If you have a health plan through a job, you can use a Flexible Spending Account (FSA) to pay for the Fisher Wallace Stimulator® with pre-tax dollars.

Private Insurance

Many private insurance companies, such as Aetna, United Healthcare and Blue Cross often reimburse patients for the purchase of a Fisher Wallace Stimulator® when it is prescribed for the treatment of pain using the procedure code E0720.

A letter of medical necessity may be required to obtain coverage. You may download a template of this letter. Your doctor will need to edit the template to reflect your particular situation.

When our device is administered by the healthcare provider, the following CPT codes apply for coverage:

Initial Office Visit (assessment only) / Code 99215 (office visit for established patient): Patient is assessed and provided with a self-assessment symptom tracking document.

Office Visits for Treatment / Codes 99213 or 99214 (office visit) plus two billings of 97032GP (electrical stimulation, manual, 15 min) per visit to cover the 20-minute treatment session plus set up and breakdown. Ideally, a minimum of nine visits occur within a two-week period (five consecutive days, followed by four consecutive days).

Concluding Assessment / Code 99215: If the patient reports significant improvement and no significant adverse events, he/she may be provided with a device for use at home using code E0720NU or E1399. A letter of medical necessity may be required to obtain coverage. You may want to download a template of this letter. Your doctor will need to edit the template to reflect your particular situation.

Fisher Wallace Laboratories is not in network with any insurance.

Fisher Wallace Laboratories does not process reimbursement paperwork on behalf of its customers, but will assist customers throughout the process.

Medicare

Medicare patients qualify for a $100 discount on the purchase of a Fisher Wallace Stimulator®. Medicare may reimburse patients for the purchase of a Fisher Wallace Stimulator® when it is prescribed for the treatment of pain using the procedure code E0720. A letter of medical necessity may be required to obtain coverage. You may download a template of this letter. Your doctor will need to edit the template to reflect your particular situation.

For Patients with Secondary Insurance: If you have a secondary insurance plan in addition to Medicare, that plan may cover the device. To apply for coverage from your secondary plan, you first need to receive a denial of coverage from Medicare. To receive your denial, submit this 1490S Medicare form. In addition to the form, you will need your prescription, proof of payment and a cover letter stating that you are requesting a denial. All information needs to be sent to the Medicare Durable Medical Equipment address in your state. The address for each state is listed on the form. Once you receive a denial from Medicare, you can submit it to your secondary insurance carrier.

 

Tricare for Life

TRICARE For Life will now cover our medical device in full if it is first rejected by Medicare. The device is covered for any veteran -- or immediate family member of a veteran -- who is either (1) retired from the armed services and over 65 or (2) on permanent disability and under 65.

Use the Medical Code: E1399 NU and the Provider Number: 1497045215, both of which are required for submission. Submit your claim directly to TRICARE For Life.

Medicaid

Depending on the state, Medicaid may reimburse patients for the purchase of a Fisher Wallace Stimulator® when it is prescribed for the treatment of pain using the procedure code E0720. New York State Residents with Medicaid coverage may be able to secure coverage if they have a secondary insurance plan that works with Integra Partners. Integra Partners is a full-service network connecting Orthotics and Prosthetics (O&P) and Durable Medical Equipment (DME) providers. You can find insurers that work with Integra Partners using this form. When requesting coverage, make sure your prescription is written for chronic pain, and use E1399 (miscellaneous electrical stimulation) as the equipment code.

The device is effective for most patients within the first two weeks of daily use, but some customers may require three or four weeks to experience results. If you need a few extra days - or even a couple of extra weeks - just call us or email us and we will gladly extend the refund period.
