Getting your free online prescription
A prescription is required in order for us to ship a device to you. You can ask your doctor to provide a prescription via this form, or you can go through the simple process that occurs during checkout.
To get a free online prescription, simply answer 8 questions to determine eligibility for device usage, then proceed with your purchase. Your answers will be confidentially reviewed at a later time by a provider licensed in your state, who will issue the prescription to our team. Proof of ID may be required.
Note: New Jersey and Arkansas state law requires a phone consultation for each prescription. If you reside in these states, you will receive a text/email to schedule a short phone call with a telehealth provider at no added cost.
