Insurance Payment & Reimbursement

We offer Medicare patients a $100 discount on the purchase of a Fisher Wallace Stimulator.

Many private insurance companies, such as Aetna, Cigna and Blue Cross will reimburse the purchase of a Fisher Wallace Stimulator when it is prescribed for the treatment of pain using the code E0720NU.  When the device is authorized only for the treatment of depression, anxiety or insomnia, the reimbursement rate decreases significantly - when the device is authorized for depression / anxiety / insomnia AND pain, the patient will be able to have the device reimbursed more easily.  

Fisher Wallace Laboratories does not process reimbursement paperwork on behalf of its customers, but will assist customers during the process.  The following information is often required by insurance companies to process a reimbursement (or payment) request:

DEVICE REIMBURSEMENT (PROCEDURE or HSPCS) CODE: E0720 

The customer needs to call their insurance company, provide them with the device reimbursement code (procedure HSPCS code) and the diagnosis code, which should be supplied by their healthcare professional and present on the written authorization. The insurance company then typically sends a DME (Durable Medical Equipment) claim form which needs to be filled out with all the above mentioned information and attached to the proof of payment. When you purchase one of our devices, you will receive an automatic email receipt that includes our Federal Tax ID #, out NPI # and the appropriate reimbursement code for our device. 

Fisher Wallace Laboratories is NOT in network with any insurance. This would be an out of network coverage. Also, you can not purchase the stimulator from any other DME supplier as we are the only ones to sell it.

NOTE:  Patients who encounter difficulty receiving insurance approval may enhance their claim by submitting a letter from their doctor that describes the necessity of the device.  You may download a template for this letter by clicking HERE.   Your doctor will need to edit the template letter to reflect your particular situation. The letter should be signed by your doctor and printed on the doctor's professional stationary. 


MEDICARE

We offer Medicare patients a $100 discount on the purchase of a Fisher Wallace Stimulator.

Medicare will currently not reimburse patients for the purchase of our device.  If you have a secondary insurance plan which covers the device, first you will need to get a denial from Medicare for coverage in order to apply for reimbursement from the secondary insurance. To do so please call Medicare and ask for the 1490S Medicare form that you will need to fill out, attach your prescription, proof of payment and a cover letter stating that you are requesting a denial. All that information needs to be sent to the Medicare Durable Medical Equipment address in your state (call Medicare to obtain that address). After you have received a denial from Medicare, you will be able to submit to your secondary insurance.
